LMI surveys

The below survey was mailed out to all households a couple months ago. These are SUPER important for everyone in Meadville to complete and return. Without these completed surveys, we are unable to apply for grant funding to make the desired street improvements. If you need another copy, please call Bobbi at 6602149070 or send an email request to citymdvl@grm.net and one will be forwarded to you to complete. 
BE SURE to write your address or utility account number on the survey for tracking purposes only. This is not an effort to take your confidential information. We do have to show the governmental entities that we've not received duplicate surveys.
